She plays a key role in shaping and delivering innovative primary care at scale, supporting services that improve access, enhance collaboration between practices, and strengthen Primary Care Network development.
Jolene has extensive experience in leading service transformation programmes, improving integration across health providers, and streamlining clinical systems to reduce variation and improve patient outcomes.
Her leadership is focused on building stronger, more connected and sustainable primary care services that better support both patients and practice teams.
Julie Kenyon
Chief Pharmacist & Managing Director
Julie Kenyon is Co-Managing Director and Chief Pharmacist at Local Primary Care, bringing more than 30 years of NHS leadership experience across commissioning, provider, and primary care settings.
She leads work to transform medicines and pharmacy services across General Practice and Primary Care Networks, with a strong focus on workforce development, service integration, and medicines optimisation at scale.
Julie has been instrumental in developing innovative pharmacy deployment models across multiple PCNs, helping to modernise how pharmacy services are delivered within primary care.
Her leadership also extends to tackling health inequalities and improving consistency and quality of care across diverse populations.
Dr Mohammed Umer
General Practitioner & Clinical Director
Dr Mohammed Umer is the Clinical Director for the Primary Care Networks of Blackburn with Darwen and Chair of Local Primary Care Federation. In these roles, he provides strategic leadership to strengthen collaboration across GP practices, championing innovation and integrated care at scale.
Alongside his leadership roles, Dr Umer is a GP Partner at Darwen Healthcare and Caldicott Guardian for the practice, ensuring patient confidentiality and data protection. He is known for his compassionate and inclusive leadership style, with a strong focus on teamwork, and has pioneered many at scale primary care services improving equitable care for the population. He also serves as the local Place Clinical and Care Professional Lead for the Integrated Care Board, working to align services and improve outcomes for local communities.’
Dr Ridwaan Ahmed

Dr Ridwaan Ahmed is Clinical Director of the North Primary Care Network and Lead for Population Health and Health Inequalities, where he drives collaborative, forward-thinking approaches to improving outcomes and reducing health disparities across local communities.
He works across GP practices and system partners to strengthen services, improve access, and embed more integrated, population-focused models of care. His work focuses on practical innovation that translates strategy into meaningful change at community level.
Alongside his network leadership role, he is a GP Partner and Principal at his practice, and Safeguarding and Prevent Lead, with a long-standing commitment to safe, equitable, and high-quality care.
Dr Pervez Muzaffar
General Practitioner & Clinical Director Darwen Primary Care Network
Dr Pervez Muzaffar is Clinical Director of Darwen Primary Care Network, where he leads the development of integrated services across local GP practices, helping to shape more joined-up, accessible care for patients.
Alongside this, he is Clinical Lead for ELMS Out of Hours Service, supporting the delivery of safe and responsive care across East Lancashire and Blackburn with Darwen beyond core hours.
A GP Partner at Darwen Healthlink since 2004, Dr Muzaffar brings extensive leadership experience and continues to play a key role in driving improvements in public health, championing screening, immunisation, and community wellbeing.
Dr Adam Black

Dr Adam Black is Clinical Director for Blackburn with Darwen East Primary Care Network and a GP at Shadsworth Surgery, where he continues to play a key role in delivering high-quality, patient-centred care.
Raised in the close-knit community of Orkney, Dr Black trained in Glasgow before undertaking three years of voluntary medical work in India, experiences that shaped his broad and values-driven approach to healthcare.
With experience in NHS management and service development, including work within the Clinical Commissioning Group and the opening of Bentham Road Health Centre, Dr Black focuses on strengthening local services and improving care for patients across the community.
Dr Hussain Moosa

Dr Hussain Moosa is a GP Partner in Blackburn and Clinical Director for the West Primary Care Network, where he leads collaborative work to strengthen and integrate local primary care services.
He also holds a key leadership role as Palliative and End of Life GP Lead for Pennine Lancashire and Lancashire & South Cumbria, guiding the development of compassionate, high-quality care across complex clinical pathways.
With roots in Blackburn and medical training completed across the North West, Dr Moosa brings both local understanding and regional leadership to his work.
