A Highly Skilled Team
Delivered primarily through weekly sessions via secure platforms such as Microsoft Teams, and supplemented by occasional face-to-face visits where appropriate, LPC brings together a highly skilled and collaborative clinical team serving care homes across Blackburn with Darwen. This includes GPs, Advanced Nurse Practitioners, Physician Associates, Clinical Pharmacists and, where available, Specialist Care Home Nurses. Together, they take a holistic approach to resident wellbeing, identifying potential issues early and responding promptly to evolving clinical needs.
Our ward rounds support continuity of care, reduce unnecessary hospital admissions and help residents remain comfortable and well cared for in their familiar environment. We actively welcome the involvement of relevant members of the wider multidisciplinary team and encourage residents’ families and carers to participate, ensuring care decisions are well informed and person-centred.
To keep everything running smoothly, our dedicated Care Coordinators oversee scheduling and act as the main point of contact for each ward round. This ensures clear communication, efficient organisation and a seamless experience for care homes, clinicians, residents and families alike.
