Local Primary Care is looking for a dedicated Care Coordinator to support GP practices across Blackburn and Darwen. In this role, you will work closely with patients, clinicians, and community services to help coordinate care for individuals with ongoing or complex health needs. You will act as a key point of contact, ensuring patients receive the right support, guidance, and access to appropriate services.
The successful candidate will help manage patient referrals, assist with care planning, and support communication between healthcare professionals and patients. Strong organisational skills, empathy, and the ability to work within a multidisciplinary healthcare team are essential.
